ABSTRACT
Objective: Although Pperioperative immune-enhancing enteral formula
(IEEF) is effectively to decrease reduces the rate of infectious
complicationscomplications that are infective, but whether chronic use
of IEEF areis beneficial is not unknown to us. A Pprospective
randomized clinical trial was performedundertaken. This was done in
order to examine the safety and effectivenessimpact of the long-term
IEEF on the nutritional condition and immunological status of non-
surgicalery patients under total enteral nutrition through the
gastrostomy access route. A total of 30 patients was randomly asigned
to two groups.
Methods: This study involved 30 patients who were randomly divided
into two groups: Two groupsone recieiveding total enteral nutrition
with either with immune-enhancing enteral formula IEEF (Group IEEF, n
= 15) orand the other with a regular polymeric enteral formula (Group
C, n = 154) for 12 weeks.wks. Nutritional and immunological
parametersimmunological variables were periodically examined
periodically.
Results: Serum concentrationsA significant increase of serum levels of
insulin-like growth factor-Ia growth factor similar to insulin,
(IGF-I), increased significantly was noted in Group IEEF throughout
the course of the study. WhileFurthermore, serum levelconcentrations
of dihomo--linoleic acid in Group IEEF was significantly decreased,
serum levels of eicosa-pentaenoic acid and dochsa-hexaenoic acid were
significantly increased. Serum arginine and ornithine concentrations
in Group IEEF wereand those of eicosapentaenoic and docosahexaenoic
acids increased significantly in the group increased. Increases of, as
did the concentrations of serum arginine and ornithine. The CD4/CD8
ratio and NK natural killer cell activity were observedalso increased
in the Ggroup IEEF, but the difference was differences were not
significant. A significant increase ofThe B-cell fraction increased,
and the decrease of T-cell fraction of peripheral lymphocytes were
observeddecreased in Group IEEF. There was noNo infectious andor
non--infectious complications occurred during the study period in both
groupseither Group IEEF or Group C, except for a significant increase
in serum BUNurea nitrogen and uric acid concentrations.
Conclusion: The Llong-term use of IEEF is safely performed in
non-surgicalery patients, which enables to cause and results in a
significant increase of in the serum IGF-I concentration associated
with increase in increased humoral immunity.
