Data Protection Act 2018 – Subject Access Request Form
Under the Data Protection Act and GDPR you have the right to ask for a
copy of the information we hold about you. You are only entitled to
make a request for personal information on behalf of another living
adult if you have authorisation to do this. Parents or guardians are
allowed to make a request for personal information on behalf of their
children, although if the children are aged 13 or over then they must
give their written permission for this.
We need some details from you to help us to find the information that
you are interested in and to deal with your request as efficiently as
possible. We also need proof of identity, as we need to be satisfied
that you are entitled to the information.

Question 4 - What documents can you send or produce to confirm the
identity and address of the Data Subject?

To enable the Council to process the request we require 2 forms of
identification to establish the identity of the Requester and the Data
Subject as follows:

One from list A - name and date of birth; and,
*
One from list B - name and current address (dated within the last
3 months)
Examples of the types of documents that are accepted are listed below,
please note copies of the documents will be accepted and originals
should not be sent. Please tick the appropriate boxes to indicate
which documents you have enclosed.
List A (Name and date of birth)
Full valid Driving Licence.
Birth Certificate or Certificate of Registry of Birth OR Adoption
certificate.
Valid current Passport.
List B (Name and Current address)
Gas, Electric, Water or Telephone bill in Data Subject’s name within
the last 3 months.
Bank, Building Society or credit card statement in the Data Subject’s
name within the last 3 months.
Letter to the Data Subject from a Solicitor/Social Worker/Probation
Officer within the last 3 months.
Additionally, please note that if your request for information is in
relation to a child aged 13 and over, please provide their full birth
certificate to confirm parental responsibility and the child’s written
consent to enable us to proceed.
Formal Declaration
In exercise of the right granted to me under the terms of the Data
Protection Act 2018, I request that you provide me with a copy of the
Personal Data about the Data Subject which you process for the
purposes I have indicated above.
