CREMATION AUTHORIZATION
I (we) hereby authorize and direct the Crematory, subject to its terms
and conditions (see below), to cremate the remains of:
(Name of Decedent)
My (our) relationship to the decedent is that of:
______________________________________. Upon my (our) oath, and under
penalty of perjury, I (we) hereby swear and affirms that to the best
of my (our) knowledge there is no other person having a prior right to
give the authorization per Oregon Revised Statutes 97.130 and to
control the remains of the above named decedent except for:
_______________________________________________________________who has
given me (us) written or telegraphic instructions to sign this
authorization. Also, I (we) hereby agree to hold the crematory, the
funeral service practitioner, Ocean View Funeral & Cremation Services;
their officers and employees harmless from any liability, cost, and
expenses resulting from the granting of this authorization. It is
fully understood by all parties signing this document or that have
granted written or telegraphic instructions that the cremation process
is subject to the following terms and conditions.
DISCLOSURES, TERMS AND CONDITIONS
1.
The body presented to Ocean View Funeral & Cremation Services and
the Crematory is that of the above named decedent.
2.
Decedent has ____ has not____ been fitted with a heart pacemaker.
If the decedent has been fitted with a heart pacemaker,
authorization is granted to remove and dispose of the heart
pacemaker in such a manner as crematory or funeral establishment
determines.
3.
For sanitation purposes, it is the policy of the Crematory that
the body be placed in a rigid container. Fiberglass, copper, and
bronze caskets are not accepted for cremation.
4.
All prosthesis (hip, joint, surgical pins, etc.) dental
bridgework, fillings, inlays, jewelry, and other similar items
will loose their identity and will be discarded.
5.
Pulverizing of the cremated remains by crushing and grinding is
part of the normal process involved in preparing the cremated
remains for containerization.
6.
The bulk of the cremated remains will be returned; however some
will become irreclaimable during cremation, processing, and
containerization.
7.
The amount of processed cremated remains may exceed the capacity
of the urn or temporary container. Any excess cremated remains
will be placed in a separate container and will accompany the
primary urn or container when released.
8.
Person(s) authorizing cremation shall at their sole expense, agree
to defend, hold harmless and indemnify the Crematory and Ocean
View Funeral & Cremation Services, its officers, directors,
employees and agents from any claim, liability, suit, cause of
action, costs or expenses (including, without limitation,
reasonable attorney’s fees and costs incurred) resulting, in any
way, from reliance on or performance consistent with the
directions, declarations, representations, authorizations and
agreements herein, including, but not limited to, any delay in or
damage arising from the transportation of the decedent’s body or
cremated remains.
9.
If shipment of the cremated remains is required, I (we) direct
such shipment via US Mail, return receipt requested.
