Nomination Form and Guidance

Please download and complete the following nomination form to be
considered for the 2012 Rick Rescorla National Award for Resilience.
To help facilitate consistency in the nomination and selection
process, keep in mind the points below when preparing your submission:

Non-governmental individuals residing legally or organizations
based in the United States or its territories may submit an award
nomination form on behalf of themselves or another. Each nominee
may only submit one nomination package, or have one submitted on
his, her, or its behalf. Nominees should focus on describing in
detail how their activities and accomplishments further the
resilience goals set forth in the award criteria contained in the
award announcement.

Please combine the nominee’s best accomplishments, initiatives, or
programs into one award nomination package. The accomplishments,
initiatives, and program activity to be considered for this award
must have occurred in calendar years 2010 - 2011.

The narrative and supporting information provided by nominees to
DHS in connection with the Rick Rescorla National Award for
Resilience may be used by DHS to educate and inform the public on
the importance of resilience to our Nation and may be used to
illustrate excellence in the area of resilience.
*
Supplemental materials are welcome but not required. The
nomination may include up to three supplemental enclosures in
addition to the nomination form itself. For example, letters of
commendation or support from local citizens, public safety
officials, or governmental officials could be appropriate. Other
materials supporting the nominee might include media coverage or
other forms of public recognition.
*
Nominees should keep the narrative of their accomplishments brief.
No more than two pages will be accepted, using Times New Roman
Font-size 11, or an equivalent. While the narrative section in the
form may appear to be only half a page long, the form will
automatically expand to accommodate the full length of your
narrative. Please be concise.
*
The total size of the email containing the nomination and other
enclosures cannot exceed 10 MB. Even short videos should be sent
as Zip files. Please keep in mind that the awards inbox has a
limited amount of total space. Large enclosures may not pass
through to the mail box.

To help ensure consistency in assessing each nomination, when
completing the narrative portion of the award nomination form, please
use the following outline for all activities, initiatives, and/or
events:

Goals, impact, local significance, and relevance to resilience
Remember to save your work as you go. This will help to ensure that
data is not lost as you populate the application form.
Once the nomination is complete, please send it and any supplemental
enclosures to [email protected]. With all e-mails, please
ensure that “Rick Rescorla National Award for Resilience” is noted in
the subject line. A notice of receipt will be e-mailed to nominees
once DHS receives the submission. Please let us know if you encounter
any problems with submitting your nomination form.
A submitter who is nominating a person, other than himself or herself,
certifies that the individual nominee has agreed to the submittal. A
submitter who is nominating an organization certifies that he or she
is authorized to submit on the organization’s behalf.
All nomination forms and supporting materials must be submitted to DHS
beginning 12:01 a.m. EDT on April 1, 2012 and no later than 11:59 p.m.
EDT on June 1, 2012, in order to be eligible. Any updates on the Rick
