Use the table below to record your CPD hours. If you have recorded
your CPD in another format, please submit that with your statement of
experience. Your record must be comprehensive, and include, as a
minimum, the details as shown on this pro forma. You may be requested
to provide further information supporting your CPD claims at the time
of review. List the CPD by Type (I to VIII) and sub-total the hours
